Community Connect Program Initiative

The aim of the Community Connect Initiative Program is to provide support, information and referrals for individuals and families who are experiencing significant vulnerabilities and complex needs.

The Program is a FREE service which will provide a universal point of access for all individuals and families to obtain support and advocacy services as well as referrals to formal service systems.

Please phone the centre to make an appointment to access the Community Connect Worker as appointments fill quickly.
